League forms are available in the various formats listed below:-

(please note that the use of macros (VBA) has been eschewed, to avoid compatibility and protection setting conflicts)

PDF .pdf Opens in a separate window, can be copied to your hard drive, not generally editable with free software. To print off forms, which you fill in and submit manually.

Word .doc Select "Save" option > "Save in"; to install to a user folder on your hard drive. To complete forms electronically for e-mail submission.

HTM .htm To complete forms electronically for e-mail submission.
